Targetting doesn't have an immediate effect, but it sets up Doom to use Demon Rage on the chosen target in the next turn. This attack is four times as strong as Doom's already powerful basic attack, and cannot be avoided. If the chosen target is no longer present for any reason, a random new target is chosen.
